Giving Perineal Care

Materials needed:

Procedure:

  1. Explain what you are going to do
  2. Wash your hands
  3. Obtain material listed above
  4. Spread paper towel on work area. Arrange supplies on paper towel
  5. Provide privacy ( close door, pull curtains )
  6. Raise bed to convenient working height. Lower rail on the side where you will be working
  7. Fold top bedding to foot of bed and cover client with sheet or bath blanket
  8. Help client into supine position and remove clothing from waist down
  9. Position waterproof protector pads under buttocks
  10. Drape the client
  11. Raise the side rail
  12. Fill wash basin two third's full with warm water
  13. Place basin on work area on top of paper towels
  14. Lower rail on side where you are working
  15. Fold back the corner of blanket or sheet between client's legs and onto abdomen
  16. Help client to bend knees and spread legs
  17. Put on disposable gloves
  18. Apply soap and water to the wash cloth or cotton balls
  19. Give female perineal care
    1. Separate labia
    2. Clean downward, with one stroke, from front to back. Use clean washcloth or cotton ball for each stroke. Put used cotton balls in bag or set aside used washcloth. Repeat this step until area is clean
    3. Rinse area, using same procedure as in step 19A and 19B
    4. Dry area completely
    5. Fold blanket back between client's leg
    6. Help client to straighten legs and turn on side away from you
    7. Separate buttocks and clean rectal area with toilet tissue, if needed. Wash area from vagina to anus, using clean washcloth for each stroke.
    8. Rinse area, using same procedure as in step 19G
    9. Dry area
  20. Give male perineal care
    1. A. Gently pull back foreskin, if client has not be circumcised
    2. B. While holding penis, clean tip, using circular motion. Start at the urethral opening and work outward. Repeat this step, using a clean washcloth or cotton balls, until area is clean. Put used cotton balls in bag or set aside used washcloths
    3. C. Rinse and dry area thoroughly, using same procedure as step 20A and 20 B
    4. D. Return foreskin to natural position if client is uncircumcised
    5. E. Clean shaft of penis using wash cloth with firm but gentle downward stokes. Rinse and dry area thoroughly
    6. F. Help client to bend knee and spread legs
    7. G. Gently clean scrotum. Wash skin folds carefully. Rinse and dry area thoroughly
    8. H. Help client to straighten legs and turn on side away from you
    9. I. Separate buttocks and clean rectal area with toilet tissue, if needed. Wash from scrotum to anus using clean washcloth for each stroke.
    10. J. Rinse area using same procedure as in step 20I
    11. K. Dry area
  21. Remove soiled bedding, washcloths, and waterproof protector pad and place in laundry container to be washed
  22. Remove gloves and discard into bag
  23. Straighten bedding; remove sheet or blanket
  24. Make sure client is safe and comfortable
  25. Place bed in lowest position ( raise rail if needed )
  26. Empty and clean wash basin. Wipe off work area with paper towels and discard into bag. Return other material to their proper place
  27. Wash your hands
  28. Record what you have done. Report any unusual conditions.
