Members of the Health Care Team

Case Manager/Care Manager

  • Assesses the client's physical, emotional, social, and environmental needs
  • Develops a plan of care to meet these needs
  • Recommends service of other professionals
  • Reviews or reassess client's progress and adjusts plan as needed

    Registered Nurse

  • Assesses, plans, and implements nursing care required
  • Carriers out doctor's orders
  • Teaches clients and family members about care techniques and procedures
  • Supervises care given by home care aids and licensed practical or vocational nurses

    Nurse Specialist

  • Gives complex nursing care to clients with special needs: cancer care, care of children, care of infants.
  • Teaches other nursing staff about new procedures
  • Acts as an advisor to other agency members about clients with special needs

    Licensed Practical/Vocational Nurse

  • Gives nursing care under the supervision of an RN
  • Cares for clients whose conditions are stable and do not require complex nursing care

    Home Care Aids

  • Provides personal care according to the clients care plan
  • Performs required housekeeping services
  • Works under supervision of an RN, LPN, or Therapist

    Physical Therapist

  • Assesses the clients muscle strength, balance, and movement of joints
  • Plans and implements the physical therapy program
  • Teaches the client and family exercises to strengthen muscles and improve movement
  • Supervises care given by home care aids

    Occupational Therapist

  • assesses movement of arms, hands and fingers
  • determines how well the client is able to perform activities of daily living (ADL), such as eating, bathing and dressing
  • supervises care given by home care aids

    Respiratory Therapist

  • Assesses client's ability to breathe with ease
  • Plans and implements the respiratory therapy program
  • Explains proper use of breathing equipment
  • Checks equipment for proper functioning
  • Supervises the care given by home care aids

    Social Worker

  • Determines the needs for financial help
  • Assesses the needs for community services
  • Arranges for theses services to be provided
  • Sees that the client and family receive the needed help

    Speech Therapist

  • Assesses the client's speech and language
  • Plans and implements programs to help client communication
  • Teaches exercises for tongue, mouth, and face
  • Teaches family how to help the client who has difficult speaking and swallowing

    Dietitian

  • Teaches the client and family about the types of food needed to maintain or improve health
  • Teaches proper menu selection and preparation according to diet ordered by doctor
