Changing a Diaper

Material needed:

Procedure:

  1. Wash your hands
  2. Obtain material listed above
  3. Put on gloves
  4. Place baby on changing surface near material. Protect baby from rolling off surface
  5. Open soiled diaper
  6. Wipe genital area with front of diaper. Wipe from front to back
  7. Roll diaper so the urine and feces are inside. Set aside for later disposal
  8. Wash the perineal area with soap and water or use baby wipes. Rinse and dry
  9. Give cord/circumcision care according to the care plan
  10. Raise baby's legs. Place clean diaper in place
  11. Fasten diaper in place. Keep diaper below umbilical stump.
  12. Apply plastic pants, if worn
  13. Dump the feces from the diaper in the toilet. Throw diaper in the trash and place in proper location
  14. Place baby in safe and comfortable position
  15. Remove and discard gloves
  16. Wash your hands
  17. Record what you have done and report any abnormal conditions to the mother or nurse
