Taking Respirations

  1. Continue holding client's wrist after taking the pulse
  2. Do not tell the client that you will count the respirations
  3. Count each rise and fall of the chest or abdomen as one respiration
  4. Count respirations for one minute
  5. Observe for the following:

    Deep or shallow breathing
    Painful or difficult breathing
    Noisy breathing

  6. Record respirations on paper, noting information listed above
  7. Assist client, as needed, into desired position
  8. Wash your hands
  9. Record respirations on client's record
  10. Report any abnormal respirations to the nurse or family members
