Report to Your Supervisor Changes in Vital Signs

Temperature:

  • Increased (Fever)
    Oral - Above 99.6
    Rectal - Above 100.6
    Axillary - 98.6

  • Decreased
    Oral - Below 97.6
    Rectal - Below 98.6
    Axillary - 96.6

    Pulse:

    Below 60 beats a minute
    Above 100 beats a minute
    Changes in the rhythm or force of the beat
    Skipping beats

    Respiration:

    Below 12 respiration per minute
    Above 20 respiration per minute
    Changes in pattern or depth of respiration
    Difficulty breathing
    Noisy breathing

    Blood Pressure:

    Any blood pressure reading that is not within normal range or is changed from what is "normal" for your client
    According to instructions in the care plan
