Assisting With Transdermal Discs

Material needed:

Procedure:

  1. Explain what you are going to do
  2. Check care plan and medication sheet
  3. Wash your hands
  4. Obtain material listed above
  5. Provide privacy (close door, pull curtains)
  6. Help client to wash hands
  7. Check label on disc container for:

    1. Right drug
    2. Right dose
    3. Right client
    4. Right route
    5. Right time

  8. Have client to remove and discard old disc into waste container. Wash skin that had been covered by old disc
  9. Ask client to select new site for new disc usually the chest or upper arm ( any area without hair is the best in this region )
  10. Observe as client applies new disc to skin surface. Be sure that the medication surface of the disc is not touched with ungloved fingers. ( your skin may absorb some of the drug )
  11. Discard disc wrapper and other used material
  12. Have client record medications taken. Personal care attendant care record if needed
  13. Store material and medication in the proper location
  14. Wash your hands
  15. Record what you have done. Report any abnormal conditions to the nurse or family member
