Assisting With Rectal Suppositories

Material needed:

Procedure:

  1. Explain what you are going to do
  2. Check care plan and medication sheet
  3. Wash you hands
  4. Obtain material listed above
  5. Provide privacy (close door, pull curtains)
  6. Assist client into bed and Sims' position
  7. Remove clothing to expose anal area
  8. Check label on suppositories for:

    1. Right drug
    2. Right dose
    3. Right client
    4. Right route
    5. Right time

  9. Unwrap suppository
  10. Apply water soluble lubricant to suppository. Do not use petroleum jelly because it is not water soluble
  11. Give glove to client to put on
  12. Hand suppository to client to insert into rectum. Guide client's hand, if needed
  13. Observe as client inserts medications and wipes anus with toilet tissue
  14. Have client remove and discard glove into waste container
  15. Assist client to wash hands
  16. Discard used material in waste container
  17. Have client record medication taken. The caregiver can record information if needed
  18. Remind client to remain on side for 15 - 20 minutes to allow suppository to melt and medication to be absorbed
  19. Wash your hands
  20. Record what you have done. Report any abnormal conditions to the nurse or family member
