Positioning Client in Sim's Position
Explain what you are going to do
Wash your hands
Provide privacy (close door, pull curtains)
Raise bed to convenient working height
Lock wheels on bed, or push bed against wall if no brakes
Lower rails on side where you are working
Lower client's head; remove pillows. Fold back top sheet
Ask client to move to side of bed nearest you. Assist client as needed
Raise side rail
Go to the other side of the bed. Lower side rail
Place one hand around client's farthest shoulder and your other hand around farthest hip. Roll client toward you, turning on side facing toward center of bed. Position the upper leg so it does not rest on the lower leg. Lower arm is behind client
Adjust pillow properly:
Small pillow under head and neck
Support upper arm and leg on pillow
Position other devices as indicated in the care plan
Make sure client is in good body alignment. Replace top sheet
Place bed in the lowest position ( raise side rails if needed )
Wash your hands and record what you have done and time
