Positioning Client in Fowler's Position

  1. Explain what you are going to do
  2. Wash your hands
  3. Provide privacy (close door, pull curtains)
  4. Raise bed to convenient working height
  5. Lock wheels on bed, or push bed against wall if there are no brakes
  6. Lower rail on side where you are working
  7. Fold back top sheet. Position client on back
  8. Raise head of bed to 45 degree angle. If bed is not adjustable, use other devices to position client ( wedge pillow, couch pillow )
  9. Adjust pillow properly:

    1. Small pillow under head and shoulders
    2. Support arms and hands with pillow

  10. Position other devices such as a footboard, trochanter roll, bed cradle, as indicated in the care plan
  11. Make sure client is in good body alignment. Replace top sheet
  12. Place bed in the lowest position ( raise bed rails if needed )
  13. Wash your hands
  14. Record what you have done
  15. Note time and position due to having to change position every two hours
