Measuring and Recording Intake and Output

Materials needed:

Procedure:

INTAKE

  1. Wash your hands
  2. Obtain material listed above
  3. Measure each remaining liquid separately after client finishes eating and drinking. Pour into a measuring cup
  4. Hold at eye level and measure the amount left
  5. Subtract the remaining amount from the amount of the original full serving
  6. Repeat for each liquid and record
  7. Rinse, clean, dry, and store container used to measure intake
  8. Wash your hands
  9. Record the amount of fluid intake and the time on the I and O form

OUTPUT

  1. Wash your hands
  2. Put on gloves
  3. Empty bedpan, urinal, commode, hat, or emesis basin into graduated container to measure the amount
  4. Measure amount of liquid and discard into toilet
  5. Rinse, clean, disinfect, dry, and store container used to measure output
  6. Remove and discard gloves
  7. Wash your hands and record fluid output and chart on flow sheet
